Provider Notes: Hello thank you for taking time to get to know me a little better My name is Mary Smith I own and operate a childcare center.

I love children I was a foster parent for 9 years so I understand the process. Im willing to do transportation in one of my Center Vans.

Visitation sessions will be held at the Childcare center on Friday through Sunday (we are closed those days).

Im willing to provide food once a month.

I have my mandated reporter my CPR the location is zoned for childcare, so I am open to the visits of the children playing with and using the toys.
